All New Members are Enrolled in our Lifestyle Foundation Program

The program is 2-6 months (based on client goals and plan) and is paid via monthly membership subscription. 

Members may pause or cancel membership subscription at any time 

(cancellation effective at the conclusion of their current prepaid month).

Monthly Membership Fees = $399

A member may add minor children under 18 for an additional $49 per month per child (to share one membership together).


A member may add adults over 18 for an additional $99 per month per adult (to share one membership together).

Monthly Membership Benefits =

4 private therapy sessions (45 minutes each)
​+ access to sign-up for optional groups (max of 4 per subscription  per month)

Terms

All Terms and Policies apply including:


A. MEMBERSHIP TERMS.

We offer a monthly membership subscription that you may choose; it is non-transferable and includes billing that occurs every (4) four weeks.  There is a monthly fee for the primary member and additional fees for sharing a single membership with other adults and/or minor children. There are no refunds for any charged subscription fees.  Any overage minutes (i.e., private session time beyond monthly membership prepaid 180 minutes) are billed in 15-minute increments @ rate of $33.25 each. There are no additional costs for optional group sessions (maximum of 4 groups per month per subscription). Some community-based group sessions may have third-party fees that client must pay (for ticket admission, etc.), to take part in the activity.


(i) Automatic Renewal

Any subscription you choose will continue and automatically renew until you cancel the membership. By choosing a recurring Membership Service, you acknowledge that such paid services have a recurring payment and you accept responsibility for all recurring charges prior to cancellation.


(ii) Cancel 

You may cancel the subscription to the service at any time for any reason. Your membership must be canceled before it renews in order to avoid the next billing cycle. 

We reserve the right at any time to cancel/terminate (or suspend for any duration) the membership of any member in the event of the following:

(a) The member commits a serious breach of this Agreement and/or our Policies, Terms of Use, Rules and Regulations. 

(b) Where any monies are due to us by the member remains unpaid after its due date for payment. 

(c) The member knowingly provides false information when initiating the membership and the false declaration(s) would have reasonably affected our decision to approve or continue the membership. 

(d) If we terminate for any reason, we reserve the right to retain any monies received to cover any reasonable costs we have incurred as a result of administration of adherence to policies and/or  collection efforts.


(iii) Unused sessions

Any sessions (video, phone and/or in-person) accrued but unused within a billing cycle will not roll over or be eligible for use after that billing cycle concludes. For example, if you commence your membership on May 1, only use 3 out of 4 private sessions (each 45 minutes in duration) for that monthly billing cycle, the additional session credit does not rollover into June (resulting in you having 5 sessions available in June). All sessions expire within the applicable billing cycle (private sessions and groups).


(iv) Changes

We reserve the right to change our subscription or adjust prices of our services only after notice to you and upon commencement of the next billing cycle. We reserve the right to vary, add or eliminate any of the particular services, treatment techniques, modalities and/or facilities provided.  We reserve the right to close or modify facility hours with, or without, notice. 


(v) Pause

There may be times when you will need to pause their membership for a period of time. When a membership is paused, the member is not authorized to use their membership services or benefits during the pause period. At the end of the membership pause term, billing will continue with the next scheduled date. All terms and conditions will continue to apply through the term of the membership pause. 


(vi) Indemnification

You will indemnify us, defend us, and hold us (and our affiliates, agents, and employees) harmless from and against any and all claims, losses, causes of action, demands, liabilities, costs or expenses (including, but not limited to, litigation and reasonable attorneys' fees and expenses) arising out of or relating to any of the following: 

(a) your access to or use of our site and/or services including any injury or loss of property while participating in services; 

(b) any actions made with your account or Account Access whether by you or by someone else; 

(c) your violation of any of the provisions of this Agreement including but not limited to (as our providers cannot diagnose any medical conditions), your responsibility to provide updated changes in your physical and mental condition and for reporting any and all injuries sustained at the time of service and you release us from any injury resulting from undiagnosed medical conditions present during your therapy treatment; 

(d) non-payment for any of the services (including Therapist Services); 

(e) your violation of any third party right, including, without limitation, any intellectual property right, publicity, confidentiality, property or privacy right. This clause shall survive expiration or termination of this Agreement.


(vii) Payment 

You confirm and agree to use only credit cards or other payment means (collectively “Payment Means”) which you are duly and fully authorized to use, and that all payment related information that you provided and will provide in the future, is accurate, current and correct and will continue to be accurate, current and correct. You agree to pay all fees and charges associated with your account on a timely basis and according to the fees schedule, the terms and the rates as published. By providing us with your Payment Means you authorize us to bill and charge you through that Payment Means and you agree to maintain valid Payment Means information in your account information.


(viii) No Services for Litigation and/or Government Action

We are a small concierge, direct care practice to provide evidence-based recreational therapy. To prevent conflicts of interest, maintain confidentiality and comply with our code of ethics, our role and scope of competency as recreational therapist provider does not include any agreement (express or implied) for any involvement (including but not limited to providing opinion or recommendation regarding a client for use in a legal matter, and/or to offer views, analysis, reports or testimony) in any legal matter. Therefore, we do not provide Recreational Therapy to any individual, relationship or small group that is seeking services in support of an legal case or who is actively involved in any legal case (there is a pending or open legal case and/or government agency action). We do not provide any services related to legal cases including but not limited to, divorce, custody, visitation, guardianship, involuntary commitment, protective orders, restraining orders, paternity, Child Protective Services (CPS), Adult Protective Services (APS), court ordered therapy (e.g., anger management, parenting, family violence, sex offender, substance use disorder, etc.), civil litigation, trust disputes, accident, workplace compensation, wrongful termination, workplace discrimination, probate or any other pending or active litigation and/or government agency action. 


(ix) Pause Membership Pending Final Resolution of Legal Matter

If litigation and/or government action arises during the course of services, we will pause the membership (stopping all services and charges) until the time of receipt of confirmation that the legal matter is fully resolved (dismissed with prejudice, exhausted all appeals, etc.) at which time the membership may resume.

Activity Addiction Awareness

ACTIVITY ADDICTION AWARENESS:

There are issues related to excessive activity engagement for some individuals. 

We request that potential clients please access health care providers who treat excessive engagement (addiction)

Inclusion Therapies is not an addiction treatment specialist (substance or behavioral). 


Substance addiction: 

We do not allow use of any substances (alcohol or drugs, including vaping) during our sessions. 


Behavioral addiction: 

Our sessions and/or groups are not intended, nor appropriate, for an individual who excessively engages in a specific treatment technique activity (behavioral addiction). These activities include, but are not limited to: an addiction to gambling, eating, exercising or dieting, shopping, video gaming, internet use, musical practice, collecting, work, love, etc.. 

If treatment staff become aware of an addiction to (or excessive engagement in) a specific treatment technique, the member will be removed from sessions and groups utilizing that activity (with non-addictive activities substituted). For example, if someone is discovered to be living with a gambling addiction involving the card game of poker, they will not engage in poker games during their treatment sessions. 


When does an activity potentially become a behavioral addiction?

  • "The essential feature of behavioral addictions is the failure to resist an impulse, drive, or temptation to perform an act that is harmful to the person or to others[.]"
  • "These characteristics include tolerance (the need to use the substance or perform the troublesome behavior at higher doses, or more and more frequently, in order to achieve the same effect); withdrawal (feelings of restlessness, irritability, and discontent following abrupt discontinuation of the substance or the behavior); obsessive thinking and planning that block out anything other than obtaining or engaging in the addictive agent or behavior; and accompanying external consequences in related to finances, health, interpersonal relationships, legal affairs, etc."
